How do we do this? Creating one health and social care system for Salford means working in partnership with others – professionals such as mental health nurses, district nurses and social workers, the voluntary sector, community groups and local people – and GP practices need to lead the way. This collaboration between all available resources will make sure that everyone gets the right support, in the right place, at the right time.
People will have better access to the advice, expertise and support they need to be as independent as they can, with the opportunity to see a wider range of health professionals and access the full range of activities in their area. Local people will require fewer hospital admissions and visits as there will be more skills and support available within the community they live.
